Liability Waiver & Contract 
I assume full responsibility for any injuries, all damages and death which may occur to me or my guests. I hereby fully and forever release and discharge A Different World Creations LLC, ADWC owners, Staff, Volunteers and/or contractors from any and all liability, claims, damages, cost, and expenses of every kind, in conjunction with the use of the facility and/or services rendered.
I, for myself and my guest agree to use all related equipment, props, activity area properly and leave them in good condition.
ADWC has taken measures to minimize the spread of Covid-19. Such measures include sanitation of supplies, having host and children wear masks, hand sanitation, encouraging social distances and educating our staff about the disease. We ask that our guests take Covid-19 precautions. When you book us, you accept any and all risk of exposure to Covid-19.
I have read and agree with the Terms and conditions.
By submitting, it will indicate that I agree to the provisions of this Release of Liability Waiver for myself and guests, and for my heirs and assigns intending to be legally bound.
